Summary :
⁣The study determines the level of importance of the risk factors identification in women in relation to risk pregnancy during the prenatal care. The survey method of a selfconstructed questionnaire was used for data collection, as well as the standardized Hospital Anxiety and Depression Scale (HADS), the questionnaire to assess coping behaviors (Brief Cope), the International Personality Item Pool questionnaire (IPIP), and the Perceived Social Support Scale (PSSS). Data collection was carried out from December 2019 to February 2020. The questionnaires were distributed to midwives working in gynaecological, prenatal, and highrisk pregnancy clinics. Out of a total of 100 distributed questionnaires, the response rate was 81% (81 returned questionnaires). We first processed the data we had obtained using the descriptive statistics method. We used the Pearson and Spearman correlation coefficient tests to interpret the statistical significance of the differences and dependencies of the variables.

Summary :
⁣Transvaginal Natural orifice transluminal endoscopic surgery (vNOTES) is increasingly being performed in gynecology practice for various of indications. We aimed to share the surgical and subjective outcomes of patients who underwent NOTES for gynecological disease. This preliminary report included 43 patients. Out of those, 39.5% had previous abdominal surgeries. 14% of all underwent hysterectomy and 86% had various adnexal procedures. Mean age, BMI and median parity were 39.6±8.9 years, 28.9±9.1 kg/m2 and 3±2 births. The mean POP-Q C level measurement was -36±14mm. The mean total duration of surgeries was 21.8±15.2 minutes. Median pain scores measured by Visual Analog Score were 4±3 and 1±2 at 6th and 24th hours of surgery, respectively. 32.5% of all patients needed one and 14% needed two additional doses of analgesic in addition to routine one postoperative dose. There was no conversion to laparoscopy or laparotomy. Only one patient had early postoperative mild complication that is minor bleeding from colpotomy and was managed conservatively. The satisfaction was measured with Patient Global Impression scale, 95.3% of all patients reported that were very satisfied at the 1st week. In conclusion, the vNOTES is a feasible approach in regards to the duration of surgery, postoperative pain scores, and cosmetic outcomes.

Summary :
⁣Hemorrhagic ascites associated with pleural effusion due to endometriosis is an uncommon presentation rarely reported in the medical literature. We present a case of a 29-year-old woman with a history of primary infertility .She presented to the emergency department for asthenia, abdominal swelling and pain. On examination she was stable with signs of massive ascites and right-sided pleural effusion. This was confirmed on ultrasound scan and computed tomography. A magnetic resonance imaging was performed to define the nature of fluid .It shows an abundant ascites (figure a) and right pleural effusion (figure b) seen as hyperintense signal on T1 weighted sequences due to hematic component. Paracentesis was done reveling a bloody ascetic fluid. She declined a pleurodesis for the pleural effusions. The patient was treated with a GnRH analogues with progressive reduction of pleural effusion This report of case draws attention to this presentation as a complication of endometriosis. This suggests that clinicians should include endometriosis in the differential diagnoses of middle-age women presenting with bloody ascites and pleural effusion in addition to infection and malignancy.

Summary :
⁣Anti-N-Methyl-D-Aspartate encephalitis is a subcategory of auto-immune encephalitis. It is known for its aggressive presenting symptoms and rapid deterioration, yet it is treatment responsive. It is associated in 50% to ovarian teratoma. Anti-NMDA receptor encephalitis with ovarian teratoma is a rare entity with rapid deterioration. Early diagnosis, surgical resection and proper medical treatment are essential for the management of this disease. We will report 2 cases of Anti-NMDA encephalitis with review of literature and discussion over treatment modalities.

⁣Our clinic was declared a COVID support in March 2020. In the first 7 months of the pandemic, 60.91% of all patients had mild forms and 3.8% had severe forms, as opposed to the last 5 months of pandemics where the percentage was of 66.51% versus 1.8%. 78.4% of hospitalized patients had elevated CRP values, 14.28% had cough symptoms, and 12.68% dyspnea. There were 5 cases in the clinic with moderate and severe forms that required transfer to Infectious Diseases and ATI.

Summary :
⁣A total of 12RCTs with 2028 women (hysterectomy: n=977 women vs E:A/R: n = 1051 women) were included. Hysterectomy was compared with only endometrial ablation in five studies and only endometrial resection in five studies and both ablation and resection in two studies. Meta-analysis found that women in the hysterectomy group were more likely to show patient-reported and objective outcomes improving in HMB comparing with EA/R (risk ratio(RR)0.75, 95%CI:0.71,0.79) and (MD44.00, 95%CI:36.09,51.91)respectively. Patient-satisfaction was higher after hysterectomy up-to 2-years follow-up, (RR:0.90, 95%CI:0.86,0.94) but this effect was absent with longer term followup. EA/R had lower risk of adverse-outcomes for sepsis (RR:0.03, 95%CI:0.00,0.56), wound infection (RR:0.05, 95%CI:0.01,0.28) and urinary tract infection (RR:0.20,95%CI:0.10, 0.42) but higher risk of fluid-overload (RR:7.80,95%CI:2.16,28.16) and perforation (RR:5.42,95%CI:1.25,23.45).

⁣186/216 women with pre-eclampsia who attended the Maternal Health Clinic met inclusion criteria. No significant differences were observed for placental morphometric measurements between women who screened as having a high vs. low lifetime risk profile for cardiovascular disease at six months postpartum. However, using multivariable modelling that controlled for maternal age, prepregnancy body mass index, gestational age at delivery, and severity of pre-eclampsia, a low placenta to birth weight ratio (<15%) was associated with an increased odds of high lifetime cardiovascular disease risk (p < 0.009).

TOA carries a high morbidity and mortality rate of 5-10% when assocaited with severe sepsis. It is a common acute gynaecological presentation for which there are no national guidelines to guide clinical decision making. Management varies based on clinical presentation and experience of the clinician, and is based on IV antibiotics (conservative management), image guided drainage, or surgical options. ⁣This was a retrospective case-control study performed in our hospital.We included all deliveries in women who had a history of bariatric surgery, operated in the same center between Jan 1, 2016 and Dec 31, 2019. This was the study group of fifty-two women. To investigate the effects of weight loss after bariatric surgery on pregnancy outcomes, we compared the study group with a control group matched for presurgery body mass index and did not undergo any bariatric procedure and delivered during the same period of time. This was the control group of 150 women. BMI was calculated using weight measured at the first prenatal visit. The main endpoints were gestational diabetes mellitus (GDM), pregnancy-induced hypertension (PIH), birth weight of the babies, age of gestation at delivery and mode of delivery.

⁣Pregnant women and their neonates represent 2 vulnerable populations with an interdependent immune system that are highly susceptible to viral infections. Maternal antibodies are produced in the setting of an infection or vaccination, and pregnant women can pass on these antibodies via the cord blood to provide passive immunity to their babies. Thus, antibodies can be measured as a marker of the immune response as well as putative protection from infection. We demonstrate that maternal antibody levels correlate with symptomatic maternal SARS-CoV-2 infection, and higher levels of maternal antibodies are associated with passive neonatal immunity. Maternal antibodies from vaccination increase with time elapsed since vaccine dose 1 and the antibodies mounted against a vaccine are detected in the cord blood of their babies. The placental transfer ratio of antibodies correlate with time elapsed since maternal vaccination dose. Taken together, these data can guide the clinical management of SARS-CoV-2 infection as well as vaccination strategies for pregnant women.

There are also many materials used for reinforcement of the abdominal wall. Women of childbearing age, often present with hernias. ⁣Superovulation is a drug-induced method to enable multiple ovulation per menstrual cycle and key component towards a successful IVF cycle. Although there are the general guidelines for dosage, the dose is not optimized for each patient, and complications, such as overstimulation, can occur. To overcome the shortcomings of this general system, a mathematical procedure and a decision support tool is developed which can provide a customized model of this stage regarding the size distribution of follicles obtained per cycle as a function of the chemical interactions of the drugs used and the conditions imposed on the patient during the cycle. Uncertainty and risk are modeled and included in optimal drug dosage decisions. This paper describes the theory, model, the optimal control procedure, and the decision support tool for improving outcomes of IVF treatment for all the four protocols used in real practice. The validation of the procedure is performed using clinical data from more than 100 patients previously undergone IVF cycles. Customized patientspecific model parameters are obtained by using initial two-day data for each patient and validated. The models are then used for predicting the customized optimal drug dosage for each patient. Two clinical trials were conducted in India. The results from the trials show that the dosage predicted by using the model is 40% less than the suggestion made by the IVF clinicians. The testing and monitoring requirements for patients using optimized drug dosage is reduced by 72%.

⁣Why are women who face poor prognoses for success in assisted reproductive technology (ART) treatment choosing to pursue procedures using their own eggs, despite receiving information that their chances of success are low? Cross-sectional study based on an anonymous questionnaire distributed to women aged between 43 and 45 years, undergoing ART using their own oocytes, at public outpatient fertility clinics and public in-hospital IVF units in Israel. The main outcome measure was personal estimation of chance to achieve a live birth after the current ART treatment cycle and the cumulative estimated rate after all the treatment cycles the patient intended to undergo.

⁣Endometriosis is a condition that affects at least 1 in 10 women worldwide and is a common cause of Dysmenorrhea, Dyspareunia, Dyschezia, and Chronic Pelvic Pain. Endometriomas affect more than 20% of women with endometriosis and are often associated with deep infiltrating endometriosis in 60-80% of women that present with Endometriomas. Management of endometriomas is extremely important because of all types of endometriosis are most likely to affect the fertility of a patient. The presence of endometriomas can cause decreased ovarian reserve and affect egg quality and implantation rates. Removal in tern can also decrease ovarian reserve and if not removed properly can result as well in high recurrence rates. This presentation will review the diagnosis and management of endometriomas including mechanisms to identify patients that may benefit from surgical management as well as advanced techniques that a surgeon may use to adequately remove endometriomas without affecting ovarian function. A discussion of tips and techniques of addressing both the endometrioma as well as the associated endometriosis will be held. Surgical techniques and types of energy used to treat endometriosis surgically will also be reviewed.

⁣Laparoscopy is regarded as the Gold Standard in diagnosis of Endometriosis. However, for deep infiltrating endometriosis (DIE), posterior compartment disease cannot be quantified until densely adherent bowel is mobilized with opening of perirectal space. This is associated with a high risk of complications needing careful planning and patient counselling. Using transvaginal ultrasound can help in the diagnosis and can be used as a screening tool followed by MRI to confirm the extent of the disease.
