Summary :
Failure rate of transvaginal cerclage (TVC) is 13%.Pregnancies will deliver pre-viable infants despite this intervention.Patients who fail Trancervical cerclages are candidates for abdominal assisted cerclages. Indicarions of transabdominal cerclages are two or more failed TVC or if TVC is impossible due to extreme shortening, scarring or laceration of the cervix.No randomized control trials comparing preconception to early pregnancy. when comparing open to laparoscopic approach, The results demonstrated that there was no differencein the rates of thirdtrimester delivery and live birth.The use of cerclage, either through vaginal or abdominal routes, to assist in delaying pre-term delivery among select women with cervical insufficiency may be beneficial, but can also carry significant morbidity.
